What Is an Integrated Care Specialist?

These mental health professionals are trained to help clients meet their recovery goals by providing assistance and guidance throughout their clients’ treatment process. They assess their clients’ needs and pinpoint where support systems are needed. They also help make plans for intervention should crises arise, and assist with developing coping mechanisms. In addition, they help link, coordinate and monitor their clients’ mental health services to ensure that no one falls through the cracks. Bear River Mental Health recognizes the importance of physical health care, and they may also help clients gain access to needed medical care as well as monitoring and assisting in coordinating those services..

How Does Case Management Work?

Case management requires good communication between the client and the integrated care specialist and that often comes from developing a relationship and creating mutual understanding. This may be achieved by beginning with a full needs assessment. integrated care specialists get to know their clients by learning about their clients’ needs, which often requires a case management needs assessment, which may include information about a person’s mental health and substance use history, their basic living needs such as food, clothing, shelter, and transportation, and any current physical health care needs.

Once a foundational relationship is built, integrated care specialists can help their clients create targeted case management goals that are individualized and meet their unique needs. Those needs may include mental health care, support and/or wrap around services, substance use intervention, crisis management and physical health care. For some people, it can be a challenge to navigate the road of mental health treatment, so having a integrated care specialist can mean the difference between success and stagnation.
